Improving the quality of primary health care and follow-up for people with diabetes

Leading a paradigm shift

ReflexD is an audit and feedback tool designed to transform practices, follow-up and care for people with diabetes.

A new way of thinking is necessary to improve the quality of health care.

The ReflexD team supports clinical teams and family medicine groups (FMG) in this process. We are collaborating with the Réseau de connaissances en services et soins de santé intégrés de première ligne (Réseau-1 Québec) and the Réseaux de recherche axée sur les pratiques de première ligne (RRAPPL).

Clinicians who want to engage in patient-centred health care use ReflexD as a reflexive analysis tool.

They learn from their patients’ trajectories, from evidence-based medicine and best practices in health care.

Audit and feedback application

The attending physician can access individual patient’s health data.

Best practices

Benchmark comparisons

A physician can compare his or her practice with that of the entire clinic.

Continuous (Quality) improvement

Standardising intervention plans within the clinical unit.

Select patients based on advanced criteria

Quickly determine a cohort of patients.

Target patients with similar characteristics, treatments or drug prescriptions.

Get a full picture of patients by analyzing their records comprehensively

Visualize the progression of laboratory results and drug prescription history in relation to HbA1c and creatinine.

Compare with practice guidelines

Analyze target patient data using validated parameters and indicators.

Use the latest test results to see if any of a patients’ active drugs are contraindicated and adjust the treatments.

A physician can compare his or her practice with that of the entire clinic

For themselves and confidentially, clinicians can evaluate their decisions and keep improving.

Set specific criteria to compare data based on treatments, lab results and drug prescriptions.

Improve the quality of care

Check if my clinical practice meets targets at the level of the FMG.

Suggest analyses that aid and improve clinical practice at the individual and FMG levels.
